Maintains current knowledge of diabetes and adheres to all processes according to the American Diabetes Association (ADA) National Diabetes Education Standards and AHD ADA Program Recognition standards. Provides optimum diabetes education to patients from adolescents through geriatric age. Requires travel between multiple hospitals and scheduled community outreach events.The value you’ll bring to the team:
Demonstrates comprehensive understanding of diabetes mellitus and gestational diabetes.
Regularly works with medical staff in multiple hospitals to provide basic and comprehensive diabetes education training and skills for referred outpatients with focus on Medical Nutrition Therapy.
Conducts initial assessment and reassessment of patient/family's education needs and develops a specific plan of care to meet those needs.
Demonstrates ability to revise plan of care as indicated by the patient's response; evaluates overall teaching plan for effectiveness in achieving optimal established goals and optimal outcome.
Utilizes and coordinates resources to provide appropriate continuum of care for the patient/family.
Provides dynamic one-on-one instruction and group instruction
Demonstrates ability to coordinate, conduct and facilitate diabetes support group meetings
Demonstrates appropriate and age-related assessment and intervention.
Develops, reviews and revises the Diabetes Program Curriculum on an annual basis or as needed to provide current and accurate information.
Obtains information and continuing education necessary to provide current diabetes/medical nutrition therapy education.
Completes required documentation as needed by the department secretary for timely and accurate patient registration and service charges
Complete documentation in the patient's medical record and Diabetes Harbor Software according to department standards and ADA Program Education Recognition Standards.
Provides timely and accurate responses to telephone calls from patients/family and other health professionals.
Understands and utilizes appropriate patient referral, scheduling and appointment processes in the department.
Able to perform computer skills required for Diabetes Education Program, Diabetes DataTrak, and email communication within department.
Member and active participant in the FHMS Diabetes Program Advisory Committee.
Recommends and develops protocols and clinical pathways to ensure quality diabetes care.
Demonstrates time management and organization skills for completing daily, weekly and long-term assignments, including traveling, classroom set-up and clean up.
Participates in community outreach events as defined by department.
Provides professional expertise to other members of the diabetes care team, nursing staff, and community.
Markets diabetes management services to the medical community and community at large.
Provides in-services and continuing education programs to meet identified/justified needs.
Adequately maintains supplies and teaching models for education program needs.
Demonstrates competence in use of office and diabetes teaching aids and equipment.
Participates in monthly PI activities to ensure quality measures are met.
The expertise and experiences you’ll need to succeed:Minimum qualifications:
Bachelor degree in Dietetics and has passed the Commission on Dietetic Registration examination
Current Florida Registered Dietitian licensure
Valid State of Florida Driver’s License
Minimum of 4 years clinical experience
Current in Basic Life Support or obtains within Introductory period
Computer skills in Microsoft office and be adaptable to new software programs
Preferred qualifications:
Certified Diabetes Educator
