Applicants must complete application in office Wednesdays and Fridays, 9am - 1pm. Applicants must ensure they have all required documents as mandated by AHCA to complete application:
HHA certificate / Active CNA License
Level II background screening done
2 Employment references
1 personal reference
Valid ID/ DL / Immigration document / Passport
Social security card
Physical done within the last 6 months
In services required:
Original 4 hours HIV
HIV update (2 hours)
OSHA/HBV (2 hours)
Domestic Violence (2 hours)
Self-Administration of Medication (2 hours)
Alzheimer’s Disease (2 hours)
Elder Abuse, Neglect, Exploitation (2 hours)
CPR
