We have an additional opening for a Private Duty Home Health

Agency Scheduler/Client Care Coordinator in Plantation Florida.

Home health scheduling and client care coordination experience is required!

Starting Salary is $50,000

Monthly Stipend for health care insurance and cellular phone.

Responsible for the coordination of client visits and maintaining records and daily logs. This includes the accurate and timely communication of scheduling changes between the office, clients, and caregivers.

Responsibilities include:

Promote and exemplify company mission, vision and values at all times.

Coordinate and schedule caregiver assignments, making certain that clients are assigned to qualified staff.

Communicate directly with caregivers on a daily basis and maintain/update scheduling software.

Schedule new cases to ensure care coordination meets AHCA regulations and timely follow through with clients/families and caregivers.

Maintain current client and caregiver roster with necessary information.

Conduct new prospective client assessments and onboarding new clients.

Required Experience:

At least one year of experience scheduling and client coordination in a Private Duty Home Health Agency office environment

Skills/Other Requirements:

Ability to communicate verbally and in writing effectively.

Computer skills - Especially private duty scheduling software.

Must read, write, and comprehend English.

Level 2 background check.

Current driver’s license.
