Job Description:
Matrix Home Care is actively seeking a compassionate and skilled Certified Rehabilitation Registered Nurse to join our dynamic team in the role of Regional Director of Nursing/Catastrophic Care for our Dade/Broward locations. In this full-time position, you will be responsible for the oversight of high-quality, patient-centered care directly within the homes of our clients. The ideal candidate will possess clinical expertise, strong communication skills, and a commitment to providing comprehensive care management.
Key Responsibilities:
Home Visits and Assessments:
Conduct facility and in-home assessments to evaluate patients' healthcare needs and develop personalized care plans.
Collaborate with patients, families, and healthcare professionals to ensure care plans are tailored to individual requirements.
Care Coordination:
Coordinate and oversee the delivery of healthcare services in collaboration with the multidisciplinary healthcare professionals.
Facilitate communication and collaboration between healthcare providers to optimize patient outcomes.
Patient Education:
Provide education to patients and their families regarding treatment plans, medications, and self-management strategies.
Offer support and resources to enhance patient and family understanding and engagement in the care process.
Documentation and Compliance:
Maintain accurate and timely documentation of all home visits, assessments, and interventions.
Ensure compliance with regulatory standards, organizational policies, and industry best practices.
Emergency Response:
Implement crisis intervention strategies to address urgent healthcare needs of the patient.
Qualifications:
Current certification - Certified Rehabilitation Registered Nurse(CRRN) is required.
Bachelor's degree in nursing or a related field.
Minimum of 3 years of experience Workers Comp, Home Care, Community Health, or a related Healthcare setting.
Strong clinical assessment and critical thinking skills.
Excellent communication and interpersonal skills, especially in a field/home setting.
Knowledge of regulatory requirements and standards for home care services.
Range $90k-$95K
