Dosage calculations are accurate and clearly displayed for pharmacist verification.
Labeling is complete with accurate expiration dating included.
Batch records are completed accurately with appropriate quantities approved.
Chemotherapy preparation is checked before admixture and strict adherence to policy and
procedure is observed.
Cleaning and decontamination of Laminar flow hoods is documented.
All intravenous admixtures are checked by a Registered Pharmacist prior to dispensing.
Pyxis training is completed with proper knowledge in stocking machines, reviewing reports,
and handling nursing concerns.
Cart fill medications are filled accurately and completely with proper documentation of
returns.
Floor stock medications are refilled and appropriate patient charged.
All returned medications and IV solutions (if appropriate) are credited in an accurate and
timely fashion. All patient charges are current.
Completes Emergency Department orders.
Trains new associates on dispensing responsibilities.
MINIMUM QUALIFICATIONS
Must have National Pharmacy Technician Certification or in Lieu of this Certification a
Licensed Pharmacist Intern is required.
Must be registered by the State of Florida by Jan 1, 2010
High school diploma or equivalent is required.
Minumum of two (2) years additional education and/or experience
