Sr. Patient NavigatorThe University of Miami/UHealth Department of Neurology has an exciting opportunity for a Full Time Sr. Patient Navigator to work at UHealth. The Senior Patient Navigator (H) provides support and guidance throughout the patient’s care continuum by helping patients “navigate” through the maze of doctors’ offices, clinics, hospitals, outpatient centers, insurance and payment systems, patient-support organizations, and other components of the healthcare system. The Senior Patient Navigator also acts as a resource for lower-level Patient Navigators (H) to ensure the timely delivery of quality care and patient satisfaction.

Obtain all relevant medical records, patient history, imaging, and other pertinent information for new patient visits to ensure comprehensive evaluation and preparation.

Review patient charts of New Patient Visits (NPVs) coming from more than 20 miles away.

Coordinate with nurses or providers to order necessary tests before the visit.

Review referrals from providers for NPVs likely to need a procedure based on test results, ensuring all appropriate tests are ordered or completed prior to their visit.

Review patient After Visit Summaries (AVS) for post-visit instructions and coordinate testing orders.

Make patient follow-up calls and track no-show patients for callback.

Manage all aspects of post-visit care coordination.

Retrieve clinical-related messages and return calls within 48 hours, assist in managing UChart messages, and route prescription refill requests.

Process authorizations for medications and procedures as needed. Assist in care coordination and navigation of appointments, acting as the liaison between patients and providers.

Develop and enhance processes to improve the clinical experience for referred patients and physicians.

Assist patients and their families in obtaining referrals to specific specialists and follow up with referring providers to ensure care has been provided by the specialist.

Handle all patient requests and follow up as necessary.

High School diploma or equivalent

Minimum 4 years of relevant work experience

Knowledge, Skills and Attitudes:

Ability to communicate effectively in both oral and written form.

Ability to maintain effective interpersonal relationships.

Ability to understand and follow instructions.

Ability to process and handle confidential information with discretion.

Any relevant education, certifications and/or work experience may be considered.
