Responsibilities
This is a PRN role with Premier Concierge Care Management LLC., located in the Miami-Fort Lauderdale Area. The Geriatric Care Manager will be responsible for providing comprehensive care management services to aging clients and their families. They will assess clients' needs, develop care plans, coordinate services, and advocate on behalf of clients. This is a hybrid role, with field visits required and remote work.
Requirements
Bilingual Spanish
Case management experience required preferably in a hospital setting
Flexible schedule including weekends, Holidays, and evenings required
Ability to communicate with physicians and other healthcare professionals to facilitate care
Ability to advocate for clients’ needs
Experience working with aging populations and knowledge of aging life care principles
Excellent assessment, communication, and problem-solving skills
Strong organizational and time management skills
Ability to work effectively in a team and independently
Proficiency in computer applications and electronic medical records
Valid driver's license and access to reliable transportation
