Minimum Position Requirements:
High School Diploma or equivalent.
Valid Florida driver's license and proof of insurance.
Must be able to pass a Level 2 and a Level 1 background screening (NO Felonies)
Summary: Assist consumers in learning new skills and expanding their natural supports as to allow a decrease of the coach's role and an increase of non-paid supports. Supported living coach must be able to work well with other support service providers as well as family members. Writing and verbal communication skills necessary.
Duties and Responsibilities:
Gather, review, and summarize available information for the functional community assessment and the development of the implementation plan.
Collect baseline data, teach new skills, and develop support strategy guides based on
the implementation plan.
Provide support and assistance to consumers in tasks they are unable to complete
independently (i.e., shopping, cooking, banking, etc.)
Attend/coordinate support plan meetings and quarterly meetings with the support coordinator and consumer. Complete all required paperwork for these meetings.
Assist consumers with all third party benefits.
Assist consumers in coordination of their medical, dental, psychological, and pharmacological services
Assist consumers in selection of residence within the community based on personal preference, financial status and the supported living guidelines as addressed by the Dept. of Children and Families.
Assist consumers in accessing community resources for the purpose of developing leisure interests and encourage participation in the activities.
Provide 24 hour on call emergency services for consumers.
All other duties as assigned by the Supported Living Services Supervisor.
Must have reliable TRANSPORTATION!
Must have experience with "start up" cases.
Must be familiar with the ICONNECT system and able to complete all required documentation within the system according to regulation.
