SPECIFIC DUTIES AND RESPONSIBILITIES

Employees of the Agency for Persons with Disabilities are required to demonstrate total commitment to outstanding customer service. This includes, but is not limited to, always acting in a responsive, professional, courteous manner with the employees, customers, and public we serve at all times.

Duties and responsibilities include, but are not limited to, the following:

Receive new licensing application and review for completeness. Provide feedback to the applicant in writing for any errors, omissions or additional documentation required within 30 days of receipt of the licensing application.

Complete review of the application and approve or deny the application within 90 days of receipt of a complete application.

Complete the Central Abuse Hotline Records Search (CAHRS) form for each licensing application received.

Verify that the licensee or applicant has provided documentation to verify that the licensee or application has checked the FDLE sex offender/ predator database to determine if there are any registrants within a one-mile radius of the licensed/proposed licensed home.

At time of site visit, verity that all employees of the applicant/licensee have been notified of all registrants within the one-mile radius and have signed and dated the FDLE printout.

Ensure that all personnel records are reviewed prior to initial licensure and annually as part of the license renewal process using the Personnel Record Checklist.

Create and update all licensing information in APD systems including GRID and APD iConnect.

If the home is applying to be licensed to serve individuals with mobility issues, verify at time of initial site visit or renewal site visit that the home meets the accessibility needs of the current residents.

Notify deficiencies noted in the applicant’s documentation or observance during the site visit in writing.

Conduct a monthly monitoring visit for each home with residents using the Residential Monitoring Checklist.

Conduct a quarterly monitoring onsite visit for each licensed home with no residents using the Residential Monitoring Checklist.

Ensure that at least quarterly each group home receives an unannounced group home monitoring visit using the Residential Monitoring Checklist.

At least 1 time per year ensure the facility is visited on a weekend or outside of normal business hours (8:00am- 5:00pm).

Document Class II and Class III violations as a Notice of Non-Compliance.

Review and approve the licensee’s Corrective Action Plan to address identified deficiencies.

Document all Class I violations using the Provider Agency Action Request Form (PAARF).

Update information in GRID on a monthly basis to reflect correct client admissions and discharges for each facility visited.

Perform other related duties as assigned.

REQUIRED KNOWLEDGE, SKILLS, AND ABILITIES

Knowledge of licensing requirements for residential group homes.

Knowledge of and ability to use Microsoft Office Programs.

Knowledge of and experience in ensuring compliance with state rules and regulations

Ability to travel for work purposes.

POSITION REQUIREMENTS

Highschool Diploma or its equivalent.

Experience with monitoring or conducting quality assurance activities.

Valid Driver’s License or other efficient means of transportation to travel to community locations to conduct monitoring and licensing visits.

BACKGROUND SCREENING REQUIREMENT
