All while understanding that together we are even better.Schedule: Day Shift PRNShift : DaysLocation: Flagler and VolusiaThe role you’ll contribute:The Admissions Liaison Nurse responds to referrals to the HospiceCare program to assess medical eligibility and enroll patients into the program. Promotes program growth by fostering professional staff development in assigned facilities and educating community resources on the mission of the hospice program. Assists in the organization through issues identification/resolution, physician education and consumer education. Duties at times may fall outside normal scheduled work hours. The Admissions Liaison is responsible for following the strategic growth plan and to meet productivity levels for the department.The value you’ll bring to the team:o Assessing and admitting eligible patientso Education staff, physicians, referrals sources of Hospice criteriao Meeting productivity of admissions per dayo Documentation accuracy per Medicare guidelinesThe expertise and experiences you’ll need to succeed:· Minimum qualifications :o Registered Nurse licensed in the state of Floridao Basic Life Support certificationo Valid driver’s license, active auto insurance and reliable transportationo Minimum 2 years of nursing experience in hospice or related field· Education and Experience Preferred:o Bachelor’s of nursingo Home health care experienceo Hospice eligibility requirementso Certification of Hospice and Palliative CareThis facility is an equal opportunity employer and complies with federal, state and local anti-discrimination laws, regulations and ordinances.
