Our Communities Connected for Kids Program provides services for children and families in the child welfare system, to ensure a community without abuse, neglect and abandonment. Based out of Port Saint Lucie, FL, this position serves as the CCKids liaison to the Social Security Administration. This position also includes the following essential functions:
Assists case management with the completion of an application for SSI/SSA benefits for children that are deemed potentially eligible for benefits, or with the redirection of funds for children receiving benefits from the Social Security Administration upon their shelter into licensed or non-relative care
Completes the initial interview at the Social Security Administration for eligibility determination, redirection of benefits or application for Representative Payee
Notifies the Social Security Office to suspend or reduce payment when children are in non-eligible placements or status
Monitors children’s placements and notifies the Social Security Administration when a child is reunified, closed permanent guardianship, adopted etc.
Monitors children’s placements and notifies the Social Security Administration when a child is moved to DJJ, hospital, runaway for 30 days or more
Receives Master Trust Withdrawal Requests and supporting documentation, ensures all information is provided and forwards to Financial Manager for approval and submission
Prepares Certification for Receipt of Funds or Purchases for each withdrawal and ensures all signatures are received
Responds to Notice of Fee Assessment requests from Case Management within 72 hours
Maintains a complete client file for each eligible child (digital file)
Updates FSFN to reflect child’s SSI/SSA income
Updates Master Trust Current Balance sheet with monthly deposits/withdrawals
Submits monthly client trust balances and 6-month accounting activity to the appropriate DCM, DCM Supervisors, GAL’s Program Director in each county by the 20th of the month, for the previous month’s activity
Prepares quarterly payment request to reimburse the State Treasurer for foster board stipends due, as indicated in the state approved web-based system (FSFN)
Completes annual rep. payee accounting as requested from the Social Security Administration
Maintains up-to-date knowledge of the Social Security Administration applicable regulations, laws and procedures
Conducts Pre-Service training with new hired staff, explaining the Medicaid program, Master Trust policies and procedures and the proper channels of communication
Attends staff meetings, in-service training and other meetings as necessary as scheduled by the supervisor or as needed within the communityDenotes Essential Functions. Other related duties may be assigned as needed.
