We are a growing home care agency in the Tampa, FL area seeking an experienced and compassionate Medical Social Worker (MSW) to assist with the start of care assessments and ongoing visits for our clients. Responsibilities:
Conduct start of care assessments to evaluate clients' emotional, social, and environmental needs.
Develop, implement, and monitor care plans in collaboration with the care team.
Provide counseling and support to clients and their families to help them adjust to new care needs.
Assess and address any barriers to care, including financial, emotional, and logistical issues.
Ensure clients' rights are upheld and advocate for their best interests.
Provide education to clients and families on managing medical and social challenges related to home care.
Conduct regular home visits to reassess clients' needs and adjust care plans accordingly.
Maintain accurate and timely documentation of assessments, care plans, and visits.
Qualifications:
Master of Social Work (MSW) degree from an accredited program.
Active and unrestricted MSW license in the state of Florida.
Previous experience in home health care, hospice, or a similar field is preferred.
Strong clinical assessment and problem-solving skills.
Excellent communication and interpersonal skills.
Ability to work independently and as part of a collaborative care team.
Compassionate, patient, and client-focused approach to social work.
Must have reliable transportation and be able to travel to client homes.
