Your Specific Responsibilities: The Department of Health in Pinellas has a Registered Nurse Specialist vacancy in the Medical Services Program based at the Tarpon Springs Center. This position will also work at the Clearwater Health Department at least 2 days per week. The successful applicant will work with Primary Care Clients, assisting with disease management, education and counseling in addition to providing immunizations, family planning and STD services to clients. The successful incumbent will work in the Medical Services program within the scope of Agency protocols and State and Federal guidelines. The incumbent works in Primary Care as part of the medical home team and is expected to perform functions requiring independent judgment. The incumbent will maintain licensure as a Registered Nurse and provide services within those guidelines. Duties will include:

Management and coordination of medical and preventive services for clients based on need and evidence-based guidelines.

Development and monitoring of individual client care plans in collaboration with client and medical home team.

Identifying and increasing client’s knowledge regarding their medical and preventive treatment.

Utilization of nursing skills including interviewing, observation, and assessment to determine the medical needs of the client and motivate the client to self-management.

Use of nursing processes to plan for ongoing assessment of risk factors and services.

Education and counseling of basic health principals and risk behaviors, including but not limited to management and prevention of chronic diseases and their complications, communicable disease control, maternal health, family planning, nutrition, and safety with emphasis on prevention.

Case management to established or referred clients in need of nursing intervention and make appropriate referrals.

Direct services to clients including immunization, administration of ordered medications, tuberculin testing, vital signs and other testing or services which fall within the nursing practice if not completed by the nurse.

Assisting examiners with client care plan as needed.

Monitoring benchmarks for the client’s on-going medical and treatment plan such as compliance with labs, medication, and primary and specialty referral appointments.

Monitoring clients for appointments and services that are overdue and follow up as appropriate.

Monthly monitoring and follow up on monthly disposition reports for behavioral health referrals and hospital discharges.

Monitoring and follow up on reports of ER visits and hospitalizations.

Monitoring of biometric measures of the client’s disease process.

Outcome evaluation for patients with chronic diseases.

Use of the electronic health record to document client services

Performs other duties as required

Keeps supervisor informed

Attends staff meetings as required

Attends training for personal and professional growth

Meets annual training requirements

Required Knowledge, Skills, and Abilities:

Ability to prioritize responsibilities

Ability to establish and maintain effective working relationships

Skill in use of a personal computer

Knowledge of patient confidentiality laws

Use multiple computer programs and software such as Microsoft office to retrieve and/or input data.

Independent problem solving skills

Strong verbal communication and listening skills

Qualifications:

Minimum

Hold a clear and active license in the state of Florida to practice as a Registered Nurse

One year experience using an EMR

Valid Florida Driver’s License with access to private transportation or able to obtain within 30 days of hire.

One year experience working as a Registered Nurse.

Preferred

At least one year case management experience

At least one year experience working in a public health environment
