Requirements:
-AHCA Recommended Form 3100-0008, August 2010, Affidavit of Compliance with Background Screening Requirements.
-Physical therapy must be provided by, or under the supervision of, an individual who is a graduate of a program of physical therapy approved by both the Council on Medical Education of the American Medical Association and the American Therapy Association, or the equivalent, and licensed by the State. Progress notes shall be maintained and must be written in the participant's file and signed by the physical therapist as services are provided.
-PT with temporary licenses may enroll as Medicaid providers.
Years of experience: 2 years (preferred)
Knowledge and Languages: English and Spanish
Type of Contract: Full-time and Part-time.
Working Hours: 8am - 5pm
Compensation will be discussed at time of interview.
