Description: Peachtree Care Health Services is expanding our outreach program to help families access FREE Medicaid home care services. We are seeking community-driven individuals who want to make a positive impact in their neighborhoods.
You will receive a list of qualified addresses in your ZIP code — individuals likely eligible for Medicaid Waiver programs (CCSP, SOURCE, ICWP). Your role is to go door-to-door, educate families about services, and connect them with our intake team.
Already a caseworker or case manager working with the elderly or disability population? That’s a big plus! Your experience and relationships can help families get the care they deserve.
What You’ll Do:
- Visit assigned households in your community using the provided address lists.
- Educate families about Medicaid Waiver home care services.
- Share flyers, brochures, and intake forms (all provided).
- Refer interested individuals directly to our intake team — no selling required.
- Optionally expand outreach through community events or social media.
Compensation & Benefits:
$50 per qualified referral (paid weekly).
