We are seeking a dedicated and compassionate Travel Nurse to join our team. This role involves providing high-quality patient care in various Home Care settings across different locations. Responsibilities
Deliver comprehensive nursing care to patients in home community care environments.
Assess, plan, implement, and evaluate patient care plans based on individual needs.
Provide specialized care for patients with conditions such as dementia, cardiac issues, and those requiring catheter care.
Collaborate with healthcare teams to ensure continuity of care and effective communication among all parties involved.
Educate patients and their families on health management and including health coaching
Maintain accurate patient records and documentation in compliance with ICD-9 and ICD-10 coding standards.
Monitor vital signs and provide critical care when necessary.
Experience
Current nursing license with relevant certifications.
Strong communication skills and the ability to work collaboratively within a healthcare team.
Flexibility to adapt to different clinical environments and patient needs as a traveling nurse.
