Assist in evaluating patients and developing personalized rehabilitative plans. Participate in the periodic re-evaluation of patients as required. Maintain detailed and accurate clinical and daily progress notes for each patient, ensuring adherence to agency policies. Assist patients with therapeutic exercises, strengthening activities, and the use of self-help devices as directed by the PT. Collaborate with the PT to provide instructions to other health team members, including Home Health Aides and family members, regarding the physical therapy program. Engage in case conferences and staff meetings to discuss patient progress and promote effective team collaboration. Undertake any additional responsibilities as directed by the PT, Registered Nurse Case Management, or Supervisor. Requirements: Successful completion of an approved curriculum leading to an associate of arts degree in Physical Therapy. Valid licensure as a Physical Therapy Assistant in the state of Hawaii. Clean criminal background check. Current CPR certification. Duties: Provides skilled Physical Therapy services, on an intermittent basis, to patients, in their homes, under direction from the Registered Physical Therapist. Flexible scheduling. Responsibilities: Perform delegated patient care tasks under the supervision of a Registered Physical Therapist (PT). Assist with the evaluation and development of a rehabilitative plan of care. Participate in the assessment and periodic re-evaluation of patients as required. Documentation and Reporting: Prepare and maintain clinical and daily progress notes for each patient, ensuring all notes are reviewed and submitted according to agency policies. Prepare summary reports as directed by the supervising PT. Record activities, findings, and patient reactions to treatment, and report any changes to the PT and the physician. Therapeutic Interventions: Under the PT\'s direction, assist patients with therapeutic exercises, strengthening activities, and the use of self-help devices. Provide guidance on exercise programs, including patient and family education. Monitoring and Reporting: Identify and report any concerns regarding medication therapy, adverse drug reactions, allergies, or contraindicated medications to the PT. Team Collaboration and Education: Work closely with the PT to instruct other health team personnel, including Home Health Aides, and family members about the physical therapy program. Assist with the training of other staff and families to ensure effective care at home. Consultation and Communication: Participate in patient home visits and provide written feedback to the PT regarding physical findings. Engage in case conferences and staff meetings to discuss patient progress and team collaboration. Discharge and Planning: Take on any other responsibilities as directed by the PT, Registered Nurse Case Management, or Supervisor. Skills and Qualities: Strong communication skills for reporting patient progress and coordinating with the healthcare team. Knowledge of physical therapy principles, therapeutic exercises, and self-help device usage. Attention to detail for accurate documentation and patient care planning. Ability to work collaboratively in a multidisciplinary team environment. Dedication to ongoing professional development.
