Supervises day-to-day activities of community and resource coordination team members and issues related to members30% of the time working with housing providers. Must reside in target location island.Remote, and in the field about 25- of Oahu, HI.

Monitors and reviews all community and resource coordination activities including member outreach and ensuring members are connected to local resources throughout care/service

Assigns community and resource coordination tasks to the team based on state and contractual requirements and member needs

Works with community and resource coordination and care management team to ensure effective, appropriate care of the member and addressing member needs

Ensures compliance with state and regulatory guidelines and standards

Educates and provides guidance and/or resources for community and resource coordination team to address member requests/needs

Evaluates community and resource coordination team’s performance and provides feedback regarding performance, goals, and career milestones

Provides coaching and guidance to community and resource coordination team to improve community resources, outreach services and connecting members to community resources to provide necessary care in a cost-effective manner

Assists with onboarding, hiring, and training community and resource coordination team members

Performs other duties as assigned

Complies with all policies and standards

Education/Experience: Requires a Bachelor's degree and 4+ years of related experience.Or equivalent experience acquired through accomplishments of applicable knowledge, duties, scope and skill reflective of the level of this position.Pay Range: $61,200.00 - $110,200.00 per yearCentene offers a comprehensive benefits package including: competitive pay, health insurance, 401K and stock purchase plans, tuition reimbursement, paid time off plus holidays, and a flexible approach to work with remote, hybrid, field or office work schedules. Actual pay will be adjusted based on an individual's skills, experience, education, and other job-related factors permitted by law.
