Ho`ōla Lāhui Hawai`i is a FQHC, non-profit community health organization serving underserved adults and children throughout the county of Kaua'i. We are seeking a full-time behavioral healthcare provider (LCSW/CSAC) to serve our community at our Waimea medical clinic.

Primary Duties
The staff provider is under the direct supervision of the Behavioral Health Dept Director. Our providers are participants in, and leaders of, the health care team which works to support the health and well-being of patients at Ho`ōla Lāhui Hawai`i.
Job Duties
Provides direct treatment and counseling services to clients, assessing, planning and treating a wide variety of complex problems. This includes identifying immediate needs, determines eligibility for treatment and other services, and develops mutually agreeable treatment options and goals.
Treatment to include counseling clients individually, in group sessions, and with their families to assist client in overcoming BH and/or substance related problems.
Coordinates care with relevant Ho’ōla providers and community agencies in the ongoing assessment and treatment of patients with behavioral health and substance abuse problems.
Advocates on behalf of Ho’ola consumers to community resources, private and public agencies plus other relevant services.
Serves as liaison for Ho’ōla in substance abuse coalitions and community groups.
Responds to referrals from the outreach, nursing staff, providers, and HHM employees regarding social issues.
Assists in the maintenance and recording of client files and statistics within a timely manner consistent with Ho’ola policy.
Acts as imbedded Primary Care Behavioral Health Provider, delivering services when patients come to PCP visits including screening and treating patients with both medical and mental health concerns.
Identifies BH needs of the community and develops programming to meet those needs. Development of new programming to include implementation plan and funding relevant outcome data collection.
Work with professional staff on the continuous quality improvement process including administering and reporting on program outcome.
Assists clients with completing, filing, and follow up on applications or documents for health and social services.
Assist in developing curriculum for identified treatment groups and facilitate/co-facilitate groups. To include facilitating chronic disease programming in group and individual settings both during and after clinic hours.
Provides immediate supervision to Case Manager .
Skills, Knowledge, Education, Training and Experience
Cultural sensitivity to our many Hawai'i cultures is important in assisting the needs of our patients. Applicants should have an unrestricted Hawai'i professional license.
Required: Must be licensed to practice in the state of Hawaii. Must meet current CE requirements for licensure. Ability to work with multi-ethnic, multi-cultural and multi-lingual and low English proficiency patients. Participates or eligible to participate with the majority of health care plans accepted at HLH.
Preferred: Experience in community-based services and interest in traditional healing - specifically Native Hawaiian.
CSAC certification preferred but not required (we can assist you in training for your certification).
