Seeking a full time social service specialist to provide case management to our kupuna population on the island of Hawai'i (the Big Island). We are a small team of professionals working cohesively to reach our mission to support kupuna, their families and caregivers in a culture of positivity and collaboration.

Candidate Requirements:

Working knowledge of the theories, techniques and methods of case management procedures.

Working knowledge of Federal, State, County and community social service programs for which most clients are eligible; eligibility requirements, application procedures and other relevant program policies.

Skill in interviewing clients to identify deficiencies in personal, legal, social, economic, and health needs.

Skill in dealing with social services clients under challenging conditions.

Ability to work effectively with the team.

Ability to collect, organize and evaluate data and to develop logical conclusions.

Ability to correctly apply applicable laws, regulations and policies to individual cases.

Ability to use a computer to enter and extract information to accomplish research and reports.

Ability to communicate effectively, both orally and in writing.

Ability to drive, have own vehicle and auto insurance.

May be required to lift, move, and maneuver equipment and/or supplies that weigh up to twenty (20) pounds.

Minimum Qualifications:

Graduate of accredited four (4) year college or university with a major in Sociology, Psychology, Counseling, Education, Social Work, Public Health, or a related field.

Have a minimum of two (2) years' experience in serving persons with functional disabilities such as cognitive impairments.

Have a minimum of three (3) years' experience in social service interviewing, counseling, case management, and assessing individual's needs.

Must possess a valid driver's license and be able to secure a valid Hawaii Driver's license at the time of employment.

Knowledge of the long-term care service system in the community and an understanding of the challenges and long-term care needs of the elderly.

Must be culturally sensitive to various ethnic groups which are representative of the population served and must be willing to attend cultural sensitivity trainings.

Must understand the concepts of person-centered care and/or be willing to learn about this core concept which is utilized across the continuum of case management.

Special Requirements:

Must be able to provide own daily transportation and minimum personal injury protection insurance is required to perform daily work assignments.

Must be able to work at various locations throughout Hawaii County.

May be exposed to communicable diseases.

Must undergo a criminal background screening per contract compliance.
