Summary:
Carry out the mission of Kealahou West Oahu and demonstrate adherence to the agency's Policies and Procedures. The employee assists and aid participants with services needed by performing the following duties. The employee consistently demonstrates good judgment and confidentiality. Maintain a professional objective to the agency's operations and its advocacy for the benefits of the participants served.
Housing Worker: Duties to include but not limited to;
Build rapport with participants to gain trust and build relationships
General Office and Clerical tasks: typing, filing, operating office equipment
Ensure confidentiality is being upheld
Conduct and schedule regular meetings or appointments with participants to ensure all housing options are being exhausted
Coordinate services that has been identified as needs of the participants
Provide intervention and prevention support as available and necessary, to include referrals and linkages to resources and other services
Maintain positive relationship with referral agencies and landlords
Maintain participants' case records, assessments, intakes and active service plan on a regular basis; Ensure case file is audit ready at all times
Knowledgeable to the basic duties of each position within the agency
Attend meetings as directed by the Site Director/Case Management Team
Maintain Tracking Logs of participant data for reporting purposes
Build new partnerships with providers within the community
Conduct home visit at a minimum of 2 times monthly
Coordinate case conferencing with all involved service providers once a month
Minimum Qualifications and Requirements:
Associate Degree in Human Services or two years experience in human and social services field that has worked with K.W.O's targeted population
Effective interpersonal skills to coordinate with groups that will support participants
Effective Written and Verbal Communication
Excellent Customer Service Skills
Ability to type with 45wpm
Ability to identify case management priorities and advocacy
Ability to identify budgeting priorities and debt management
Ability to take responsibility for decision making
Ability to demonstrate critical thinking and problem-solving skills
Ability to present information and respond to questions from groups, clients and general public
Ability to follow instructions, perform tasked assigned, adjust work pace to accommodate work load, lift 45lbs. and walk at length in addition to climbing stairs repeatedly
Ability to be non-judgmental; Ability to make non-biased decisions and maintain confidentiality
Ability to be aware of participants' history and current activity
Ability to operate office equipment
Skills in utilizing word documents, spreadsheets and emails
Must have C.P.R. and First Aid Training
Must have current TB clearance
Must have a valid Drivers License and have access to a vehicle to carry out job duties and maintain an excellent driving abstract for insurance purposes
