Duties and Responsibilities
1. Assesses, plans, evaluates, implements and monitors resident health problems and care-related issues.
Handles emergencies independently and makes decisions related to the most appropriate intervention e.g. immediate transport, physician notification, emergency vehicle transport, CPR intervention, etc.
Conducts admission assessment and initiates admission documentation.
Oversees delivery of care to residents on unit.
Ensures staff compliance with governing regulations and facility policies, procedures, and practices.
Ensures notification and/or notifies families regarding change in resident status/condition.
Completes program assessments such as fall, aims, neurological skin, etc.
Responds to resident requests/needs
Refers to other parties/providers as appropriate.
Documents resident behaviors/conditions
Notifies medical provider regarding resident condition, as appropriate.
Reports to oncoming shift concerns related to resident status, unfinished business and other pertinent data related to unit activities.
Ensures cares are delivered and documented in appropriate manner and timeframe.
Ensures ordered labs are drawn and transported to lab in a timely manner
Completes discharge documentation.
Develops and updates individualized plans of care.
Performs duties in compliance with Standards of Nursing practice.
2. Staff Supervision/Staffing
Serves as liaison between nursing units including conflict resolution.
Serves as resource to other RNs and LPNs related to resident health assessments, medications, treatment interventions and care plan updates.
Assigns, directs, instructs, and evaluates licensed nursing staff and certified nursing staff in patient care delivery on assigned shift.
Determines appropriate staffing levels, staff assignments, and staff mix in case of call-ins and 1:1 supervision of unstable residents
Obtains replacement staff as necessary to accommodate resident and staffing needs.
Coordinates staffing with other floors to maintain minimum staffing levels.
Adjusts employee schedules as necessary to accommodate resident and staffing needs and adjusts staff assignments to provide optimal resident care.
Ensures adequate staffing during resident meals.
Evaluates need for medical assistance for employee-related work injuries/illnesses.

Reviews and signs off on resident care flow sheets.
Approves staff overtime as warranted, documents justification.
3. Documentation
Participates in facility quality assurance program by assessing program implementation activities and documents findings of these assessments.
Accurately & completely documents resident assessments & interventions in the medical record.
Documents information r/t
o Focused charting
o Alert charting
o Incident reports
o Monthly summaries
Transcribes and notes medical orders.
May utilize Resident Assessment Instrument (RAI) process.

Provides & documents resident treatments

4. Medication/Treatment Administration
Provides and documents resident treatments.
Dispenses and documents medication administration.
Consults with medical provider and pharmacist regarding medication side-effects, dosages, etc.

5. Interpersonal Relations
Makes rounds with medical provider to elicit/provide resident-specific information.
Works with members of interdisciplinary team to plan problem-specific interventions.
Consults with families to apprise of resident condition and to gather pertinent resident care information.

6. Other Duties
Performs other duties as assigned
Physical Requirements
Endurance for frequent standing, walking, bending, stooping.
Occasional squatting, twisting/turning from waist.
Limited sitting, reaching above shoulder level.
Pushing/pulling objects on rollers/wheels.
Moving objects horizontally (left to right) and vertically (up and down).
Frequent lifting up to 10 lbs., occasionally up to 25 lbs.
Ability to maintain a stable posture and gait with hands free to perform anticipated (or routine) and emergent patient care.
Frequent handling (holding, grasping, working with hands), fine finger manipulation (pinching, picking, working with fingers).
Senses needed to perform essential functions of the position:
o Ability to hear and talk in quiet/noisy surroundings and talk/hear over the phone.
o Vision for near/far acuity, depth perception, color vision.
o Ability to feel size, shape, temperature, texture.
o Ability to determine hot/burning equipment.
