Below is a summary of documents required for each Service Provider:
- Articles of Incorporation/Organization – required for corporations, partnerships, and limited liability companies.
- Articles must indicate ownership OR Business License – required for individual/sole proprietors or single-member LLCs
- Commercial General Liability Insurance minimum of $1 million
- Workers’ Compensation Insurance that complies with applicable statutory limits
- Auto Insurance with minimum liability of $300,000
