Offers may vary depending on the circumstances of each case.Summary:The Heart and Vascular Screening Program Coordinator RN works closely with leaders from across the Rush System for Health to define and implement strategies and workflows to improve lung care for the Rush community. The Program Coordinator RN functions within the full scope of practice to identify and secure support and resources necessary for successful origination and implementation of heart and vascular screening initiatives. The continuum of care will extend throughout the Rush health system, primarily serving the outpatient community. The Program Coordinator is responsible for meeting program goals in a timely and cost-effective manner. Program success will be measured through performance goals, program metrics, and other monitoring systems. The individual who holds this position exemplifies the Rush mission, vision, and values and acts in accordance with Rush policies and procedures.Other information:Required Job Qualifications: Bachelor’s degree in Nursing or Master’s degree in Nursing required. Current State of Illinois Registered Nurse licensure required. At least three years of clinical nursing experience required. Demonstrated ability to navigate a complex healthcare environment to achieve results. Excellent written and verbal communication skills required. Proficiency with the Microsoft Office Suite. Experience with electronic medical records.Physical Demands: Requires the ability to walk throughout the medical center.Responsibilities: Establishes therapeutic culturally competent relationship with Rush system patients and families through effective communication, acknowledgement of patient/family perceptions of health and illness, and mutual goal setting for optimal patient experience and outcomes. Collaborates and communicates consistently and effectively with internal and external disciplines and services to meet the needs of patients/families and program objectives. Applies interpersonal skills to maintain a positive open network of professional relationships with all members of the healthcare team through effective communication, mutual support, and respect. Coordinates care using the nursing process, multidisciplinary collaboration, and coordination of all appropriate health services and community resources across the care continuum. Ensures continuity of care between inpatient and outpatient settings. Assists management team in strategy development and implementation of program initiatives. Utilizes internal and external resources and systems to effectively meet the needs of patients, families and the organization. Participates in continuous quality improvement process within the Rush Healthcare System. Monitors patient outcomes in accordance with defined standards of care. Identifies issues and formulates solutions impacting quality and cost effective care. Responsible for maintenance of standards of care mandated by regulatory agencies. Identifies opportunities to streamline and continuously improve screening workflows. Participates in policy and procedure development and review for clinical activities. Participates in educational programs and counseling for patients, families, healthcare team, and students. Creates an environment of partnership, collaboration, seamless quality, and service enhancement across teams. Encourages ongoing learning and skill development for team members.
