Mental Health Care CoordinatorJob DetailsJob LocationChicago, ILPosition TypeFull TimeEducation Level4 Year DegreeSalary Range$65,000.00 - $80,000.00 SalaryTravel PercentageNegligibleJob ShiftDayJob CategoryProfessional ServicesJob DescriptionSTATEMENT OF PURPOSE : The Mental Health Care Coordinator is responsible for coordinating all aspects of behavioral healthcare and facilitating services for clients who have vision loss and/or other specialized support needs as part of a 2-year behavioral health initiative. This is a limited-term position lasting up to 2 years. The selected candidate will work closely with clients, families, providers, and internal and external program staff to help navigate clients to behavioral health care both within the agency and to community partners, where appropriate.TRAVEL REQUIRED : This position will occasionally require local travel (within the Cook County limits).SPECIFIC ACTIVITIES INCLUDED WITHIN THE CONTEXT OF THE BEHAVIORAL HEALTH INITIATIVE INCLUDE, BUT ARE NOT LIMITED TO:

Conduct and review intake assessments to address clients’ behavioral health and social service needs;

Conduct outreach calls to all clients who have completed an intake assessment to assess any additional needs and potential barriers to accessing care, such as transportation needs, health literacy barriers, cultural factors, etc.;

Coordinate referrals and facilitate access to appropriate behavioral health, rehabilitation, social services, and community resources;

Develop individualized barrier reduction plans to help clients overcome obstacles to receiving coordinated services;

Communicate effectively with clients, caregivers, providers, and other program staff to ensure seamless care coordination and service delivery;

Advocate for clients' needs and act as a liaison between clients and various service providers;

Educate clients and caregivers about available resources;

Initiate letter and telephone campaigns to clients "lost to care" to re-engage them back to care;

Learn and utilize electronic health record system and designated case management software to document client information, referral plans, and program activities;

Maintain thorough case management documentation and reports;

Assess need for accommodations and ensure all communications are fully accessible for clients with vision loss or other disabilities;

Attend regular meetings and provide reports on progress;

Any other duties as deemed necessary by the Supervisor.

QUALIFICATIONS:

Bachelor’s degree in social work, psychology, healthcare administration or related field.

2+ years of care coordination or case management experience preferred.

Knowledge of behavioral health conditions, community resources, and social determinants of health such as housing, transportation, nutrition, employment, etc.

Strong communication, coordination, and advocacy skills.

Understanding of assistive technologies and strategies for accessibility.

Ability to work with clients from diverse backgrounds with empathy for cultural sensitivity.

Bilingual language skills a plus.
