ALDEN VILLAGE HEALTH FACILITY is seeking a FULL-TIME CASE MANAGER. The Case Manager will integrate, coordinate, and monitor Active Treatment Programs for approximately thirty (30) residents. The qualified candidate must have a bachelor's degree in Human Services field and must have at least 1 year of experience working directly with persons with mental retardation of other development disabilities. The qualified candidate must also have at least 1 year of experience in a supervisory role. This is a facility that cares for individuals with Developmental Disabilities.
