Assessing and re-evaluating the patient’s condition and needs as required
Provide patient care according to patient’s care plan and needs
Teach the patient and/or family members about care and support skills
Communicate regularly with Clinical Care Coordinators, Interdisciplinary Care Team, and QA team
Accurately and thoroughly document patient care observations, interventions, and evaluations on the day services are rendered
Participate in and share data for Performance Improvement activities to identify opportunities for patient outcome improvement
Supervision of License Practical Nurse when applicable
Develop care plan and supervision of home health aid when applicable
Be an advocate for your patients and their families as you provide the tools and training to allow them to maintain a safe quality of life
Qualifications:
Licensed as a Registered Nurse in the state of Illinois with good standings
Valid Driver's License and reliable vehicle with current insurance
Demonstrated ability to work autonomously
Prior Nursing experience in a Home Health, Hospice, Rehab, or Hospital setting is required
Experience with Electronic Medical Records (EMR) software is preferred
Comfortable with technology and must have a smart phone or mobile device
Current CPR certification
Strong communication and interpersonal skills
Time management to balance patient care, travel, and documentation
Great customer service
