Work directly with supervising Registered Nurse

Provide patient care according to client’s care plan and needs

Teach the client and/or family members about care and support skills

Communicate regularly with RN Case Managers, Clinical Care Coordinators, and Interdisciplinary Care Team

Accurately and thoroughly document patient care observations and interventions on the day services are rendered

Participate in Performance Improvement activities as assigned. Communicate information that enable collection and root cause analysis of data to identify opportunities for clinical improvement

Be an advocate for your patients and their families as you provide the tools and training to allow them to maintain a safe quality of life

LPN Qualifications:

Licensed as a License Practical Nurse in the state of Illinois in good standings

Valid Driver's License and reliable vehicle with current insurance

Demonstrated ability to work as a team with supervising RN and interdisciplinary team

Prior Nursing experience in a Home Health, Hospital, Rehab, or ALF setting is preferred

Experience with Electronic Health Records (currently using Kinnser software)

Comfortable with technology and must have a smart phone or mobile device

Current CPR certification

Strong communication and interpersonal skills

Great with customer service
