Social Support Specialist

Summary The Social Support Specialist is responsible for providing intervention through education and referrals to community agencies to assist in the resolution of social, emotional, and financial difficulties related to the patient’s illness or disease state. Education Must have a Bachelor's degree in social work, psychology, sociology, or other field related to social work. Licensure/Certification Current licensure preferred. Must have a valid state driver's license, reliable transportation, and proof of auto insurance. Addendums are required for 0153 Nurse Leader, 0249 Registry RN, 0252 Registered Nurse, 0235 Advanced Registered Nurse, and 0236 Expert Registered Nurse. Experience At least three years of experience providing outreach, disease management, and/or self-management services to patients and families. Other Qualifications Must be able to complete job duties autonomously. Demonstrated leadership skills and strong organizational skills. Ability to interpret and analyze medical and social history information. Exceptional communication skills, both written and oral, ability to positively influence others with respect and compassion; fluency in a foreign language is a plus. Requires ability to communicate and resolve conflict. Strong work ethic built on a foundation of proactivity and team work. Ability to navigate ambiguity with the aid of structured problem solving techniques.
