Identifies educational needs, develops program(s) to meet those needs, and coordinates implementation of the program(s).

Provides direct diabetes self-management education and follow-up to patients, family members, and staff.

Reviews self-monitoring blood glucose results and discusses recommendations by physicians regarding lab work, starting or changing medications with patients.

Downloads and interprets continuous glucose monitoring system data for recommending different treatment regimens for patients.

Meets with company representatives regarding diabetes products and services.

Participates in the planning, development and implementation of professional and patient education seminars.

Works independently with limited direct supervision following State of Iowa professional licensure rules and regulations, Mercy Medical Center – Des Moines policies, and professional judgment.

Serves as a resource in area of specialty and collaborates with other disciplines.

May have the following duties as Program Coordinator:

Serves as program coordinator of the Diabetes Education Program.

Collects organizes and submits data for all reports for the state of Iowa Certification and ADA Program Recognition.

MINIMUM KNOWLEDGE, SKILLS AND ABILITIES REQUIRED :

Current license with the State of Iowa that meets the National Certification Board for Diabetes Educator (NCBDE) discipline requirements for CDE (i.e. Registered Dietitian, Registered Nurse, etc.)

Demonstrates competency within a specialized area of clinical practice.

Certification through NCBDE as a Certified Diabetes Educator obtained within two (2) years of hire.

Proof of completion of Mandatory Reporter abuse training specific to population served within three (3) months of hire.

Basic Life Support (BLS) for the Healthcare Provider certified or obtained by the end of the orientation period (approximately six (6) weeks).
