JOB SUMMARY: The purpose of this position is to provide direct care to residents, under the supervision of a licensed nurse, in accordance with facility policies and procedures and report resident needs and concerns to a licensed nurse.
JOB DUTIES (include but not limited to):
1. Follow Resident’s Right policies at all times.
2. Follow written and oral directions.
3. Follow established performance standards and perform duties according to standard of practice, nursing service policies and procedures.
4. Assist new employees in following established facility policies, procedures according to standard of practice and state and federal regulations.
5. Identify resident problems, noticeable changes and concerns and report them immediately to your charge nurse.
6. Complete assignments timely, completely, accurately and in a cooperative manner.
7. Conduct resident rounds as assigned.
8. Initiate corrective action as necessary with direction off a licensed nurse.
9. Identify and report safety hazards and emergency situations and initiate corrective action per license staff instructions.
10. Assume personal responsibility for following facility procedures related to control of equipment and supplies within the unit.
11. Document incidents and or unusual problems according to established facility procedures.
12. Participate in interdisciplinary team and other facility and committee meetings as assigned.
13. Participate in the development of an individualized plan of care of assigned residents.
14. Review and follow care plans and perform nursing care of each resident for every shift.
15. Follow facility procedures for admission, discharge and transfer of residents.
16. Complete documentation accurately, timely and follow facility policies.
17. Listen to resident and family complaints and report to charge/treatment nurse.
18. Assure residents have call bells at hand and answer them promptly. Check on residents who are unable to use call bell periodically.
19. Assure residents are given adequate assistance with meals, nourishment and transportation to meal service areas.
20. Consistently work cooperatively as a team member respecting peers and supervisors.
21. Observe all facility safety policies and procedures.
22. Come to work in a clean, neat uniform and consistently present an appropriate professional appearance according to facility policy. Long hair needs to be tied up in a ponytail and nails should be kept short with no dark nail polish.
23. Assume accountability for data contained in the employees’ handbook to be reviewed at evaluation time and signed.
24. Observe infection control procedures/practices.
25. Must meet and maintain facility requirements for mandatory and continuing education.
26. Must attend in-service training and staff meetings. Twelve annual hours from date of hire to first year anniversary date.
27. Support the organization’s mission, vision and values.
28. Represents the organization and High View in all interactions with staff, residents and families, medical staff, other health care professionals, community organizations and the general public.
29. Performs all other duties as assigned or requested.
