Job Summary:

As the Minimum Data Set (MDS) Coordinator, you will be responsible for assuring the timely completion of accurate assessments and interdisciplinary care plans that meet State guidelines for all Maine Avita

Communities. Included is identifying resident acuity and needs, helping to determine specific care needs and communicating needs / expectations to responsible parties.

Duties and Responsibilities:

Completes Minimum Data Set (MDS) Assessments for all residents assigned using discretion and independent judgment to ensure that appropriate and accurate data is submitted.

Conducts resident interviews in compliance with Resident Assessment Instrument (RAI) requirements for MDS.

Seeks care staff feedback regarding resident condition/care to ensure accurate coding of MDSs.

Completes full comprehensive MDS Assessment for residents upon admission, annually, and with significant change.

Electronically stores and submits MDS assessments with current EHR system.

Monitors completion of MDSs by all disciplines within time frames prescribed by regulatory guidelines.

Advises leadership of incomplete and/or untimely assessments.

Advises supervisor of persistent issues related to non-compliant documentation on assigned residents.

Ensures accurate, timely completion of the MDS/CAAs/Triggers sheet for assigned residents.

Monitors and ensures completion of 7-day look-back documentation within prescribed MDS timeframe.

Participates in Care Plan meetings, as requested by RCD/ED.

Monitors and audits clinical record documentation on assigned residents to ensure accuracy and timeliness.

Promotes and participates in ongoing education related to all aspects of the MDS process, inside and outside the Community, to maintain and enhance knowledge of current / new regulations.

Will report and review the current case mix at each community to the Executive Director and Resident Care Director.

Complies with applicable legal requirements, standards, policies and procedures including but not limited to those within the Compliance Process, Code of Conduct, Federal False Claims Act and HIPAA.

Perform additional duties as assigned.

Minimal Eligibility Requirements:

Registered or Licensed Practical Nurse, completed state-required training

Requires the ability to communicate in English both orally and in writing

Previous MDS Experience required

Possess a valid Maine driver’s license

Ability to communicate effectively with residents, families, staff, community officials and the public

Must have compassion for and desire to work with the elderly

Must meet all State health requirements

Must be able to perform duties and responsibilities with or without reasonable accommodation

Must pass criminal background check
