The Department of Medicine, Division of Endocrinology is seeking a DSMT Program Manager (Sr. Nutritionist Manager) . The DSMT Program Manager is the primary Diabetes Quality Coordinator (QC) responsible for overseeing the day-to day operations of the Diabetes Self-Management Education and Support (DSMES) service at all sites. The DSMT Program Manager ensures that the National Standards (NSDSMES) are met and maintained at all times. The DSMT Program Manager supervises the quality of education by provided by diabetes educators.Specific Duties and ResponsibilitiesOversight of DSMES Services:Oversees the planning, implementation and evaluation of existing and new DSMES services.

Ensures DSMES services are evidence-based.

Liaises between the DSMES team members, the Advisory Group, and clinical leadership (Diabetes Center, Clinical Director).

Monitors and facilitates maintenance of DSMES team members’ qualification (CE credits, training, competency, licensures, and registrations).

Responsible for maintaining ADA Recognition and participating in the evaluation of the DSMES service’s effectiveness.

Ensures service design incorporates population needs.

Develops and implements telemedicine care models for one-on-one and group DSMT classes.

Ensures ongoing service evaluation and continuous quality improvement plan is reviewed at least annually.

Participates in the development of training materials.

Contributes to and participates in a continuous quality improvement process to measure DSMES program and to identify and address opportunities for improvement.

Works with clinical leadership to identify and implement strategies to increase referrals and retention of DSMES participants

Maintains 15 hours of continuing education annually specific to diabetes, diabetes related topics and behavior change and self-management education strategies.

Supervision of Educators:Supervises the quality of education provided by diabetes educators and conducts bi-quarterly reviews of employees conducting diabetes education.

Appraises performance of diabetes educators (including self) to identify areas of strength and areas for improvement and to develop a plan for improvement and growth.

Participates in peer review process to evaluate performance of other professional instructional staff.

Instruction of Program participants:Collects participant assessment data

Collaboratively develops educational goals, learning objectives, and a plan for educational content and teaching methods with DSMES program participants.

Provides educational interventions that utilize primarily interactive, collaborative, skill-based training methods and maximizes the use of interactive training methods.

Collaboratively develops an individualized follow-up plan with each program participant.

Evaluates effectiveness of educational services provided by measuring attainment of learning objectives.

Conducts a follow-up assessment upon completion of DSMES program services.

Documents assessment data, educational plan, educational services, and results in each participant’s educational records.

Communicates assessment and plan with referring clinician.

Utilizes a team approach to provide services and collaborates and communicates with team members when needed.

Identifies when a program participant’s needs are outside the scope of the instructors’ practice and arranges for additional service to meet needs.

Minimum Qualifications:Bachelor's degree in Nutrition. Additional education helpful. Certified Diabetes Educator (CDE) or Certified Diabetes Care and Education Specialist (CDCES). Licensure as registered dietician. At least 2 years clinical experience in diabetes outpatient or inpatient setting. Experience in program management.Physical Requirements:Sitting, standing and walking for extended period. Reaching by extending hand(s) or arm(s) in any direction. Finger dexterity required to manipulate objects with fingers rather than with whole hand(s) or arm(s).
