We are seeking a dedicated and compassionate Autism Waiver Case Manager to join our team. In this role, you will work closely with clients, behavior technicians, and service providers to ensure that individuals enrolled in the Autism Waiver Program receive the necessary support and services.
Responsibilities:
Provide case management services to individuals and families in need
Conduct assessments to determine client needs and develop appropriate service plans
Create, implement, and monitor individualized treatment plans tailored to each client's goals and needs
Coordinate and monitor the delivery of services to clients
Collaborate with other professionals and agencies to ensure comprehensive care
Advocate for clients and connect them with community resources
Maintain accurate and up-to-date case records
Experience:
Minimum of 5 years of experience in case management or a related field
Knowledge of workforce development programs and resources
Experience in supervising and mentoring staff
Familiarity with developmental delay and crisis intervention techniques
Understanding of behavioral health issues and treatment options
Strong background in social work or special education
Qualifications:
Must have a license in Special Education, Social Work, School Psychologist, or related fields.
Spanish language is a plus.
