Job Description:
We are seeking a compassionate and experienced caregiver to provide total care for a male patient with a range of medical
conditions requiring consistent support. The ideal candidate will have experience using mobility assistance equipment, including
the Hoyer Lift Sling and the Barton Transfer System (training provided if necessary). Both male and female candidates are encouraged to apply.

Age: 89
Estimated Weight: 150 lbs or less
Medical Conditions
o Hypertension
o Anemia
o Degenerative Disc Disease (Lumbar)
o Ulnar Neuropathy
o Chondrocalcinosis
o Arrhythmias
o Transient Ischemic Attack (TIA)
o Hyperlipidemia
o Severe Dementia
Responsibilities:
Morning Care:
Hygiene and grooming: bathing, oral care, shaving, and skincare
Toileting and incontinence care
Assistance with dressing
Daily vital checks
Provide snacks
Transfer patient from bed to a seated position using the Hoyer Lift or Barton Transfer System
Make the bed and change linens as needed
Provide companionship (games, puzzles, conversation)
Afternoon Care:
Prepare and assist with feeding lunch (simple meals such as soup, sandwiches, fruit, cookies, etc.)
Provide snacks as needed
Toileting and incontinence care
Engage patient in light exercises (as recommended by physical therapist) and activities
Monitor for fall prevention
Evening/Night Care:
Prepare and assist with feeding dinner (heat prepared meals)
Administer packaged medication (mix with applesauce)
Evening hygiene care: bathing, toileting, incontinence care
Assist with transfer from seated to bed position
Maintain a clean and tidy patient area (vacuum, dust, light housekeeping)
Additional Duties:
Maintain an inventory of food and supplies as needed
Provide a soothing environment with music (jazz is especially comforting for Client)
Qualifications:
Certifications such as: Certified Nursing Assistant (CNA) and/or Home Health Aid (HHA) are desirable but not
essential
Prior experience caring for patients with total care needs
Familiarity with using mobility aids (Hoyer Lift, Barton Transfer System) is preferred but not required (training available)
Must be patient, reliable, and compassionate
Strong communication skills
Able to assist with basic meal preparation
