Case Manager

Key Responsibilities:

Provide case management services to individuals with developmental disabilities, ensuring their needs are met, and they have access to appropriate services and support.

Develop, implement, and monitor individual service plans (PCPs) for clients.

Advocate for clients to secure necessary services, funding, and resources.

Conduct assessments to identify client needs and develop care plans in collaboration with the individual and their family.

Maintain up-to-date, accurate records and case notes in the system.

Coordinate with healthcare providers, community organizations, and other support agencies to ensure comprehensive care.

Provide education, support, and guidance to individuals and families regarding available services and rights.

Regularly communicate with clients, families, and other stakeholders to ensure quality service delivery.
