We’re Seeking Candidates Proficient In:
G-Tube and Oxygen Care
Tracheostomy Management
Ventilator Support
Maryland Board of Nursing License or Compact Nursing License Required
Key Responsibilities:
Market homecare services to hospitals, Nursing Homes, Insurance Companies, REM Waiver case Managers.
Perform pre assessment of potential nursing clients
Perform intake of new clients
Perform on-going assessments
Prepare and Review medical documentation including but not limited to:
o CMS-485/487
o Medication Administration Records
Review and approve progress notes from direct caregivers
Liaison with state and insurance companies
Coordinate and oversee individualized patient care
Develop and implement plans of care
Initiate and manage treatment plans
Recruit, supervise, and support caregiving staff
Monitor care delivery and ensure service excellence
Provide follow-up and problem resolution for clients and families
Provide staff training
