Harbor Health Services is seeking a talented Pharmacy Outreach Representative to join our Pharmacy team at our community health centers. Position is based in Dorchester, MA.

Role:
The Pharmacy Technician Outreach Representative works under the direct supervision of the Pharmacy Operations Manager and assists with prescription retention and adherence rates at the in-house pharmacies as well as generating new prescription transfers from outside pharmacies.
Responsibilities:
Creates and maintains a pharmacy patient database of all patients on five (5) or more medications at HHSI’s in-house pharmacies.
Synchronizes all HHSI pharmacy patient medications so that prescriptions are due to be filled on the same date.
Reaches out to pharmacy patients 5-7 days before their medications are due to be filled to confirm the patient’s medication list and documents any changes.
Requests new prescriptions from patient’s provider(s), in the event a new prescription is needed.
Assists with obtaining new prescriptions that require prior authorization or any other documentation.
Acts as the liaison between the patient and the pharmacies regarding prescription management.
Successfully processes prescription medications through various insurances including the error resolution process.
Coordinates the filling and/or mailing of patient’s prescriptions with the in-house pharmacies.
Under the supervision of the Pharmacy Operations Manager, strategically identifies opportunities for transferring outside prescriptions to our in-house pharmacies.
Communicates with the Clinical Pharmacist regarding any medication adherence issues.
May require occasional in-house pharmacy staffing responsibilities.
Requirements:
High School diploma or equivalent, Registered Pharmacy Technician required, Nationally Certified PhT preferred
Current BLS (Basic Life Support for Healthcare Providers) Certification required
1-3 years of pharmacy or related experience in an outpatient clinic pharmacy services or retail pharmacy preferred
Must be flexible and be able to work different schedules to meet client and pharmacy needs
Demonstrates competent working knowledge of insurance plans
Thorough knowledge of the Pharmacy Technician Procedures
Ability to work in fast-paced environment and troubleshoot and solve problems
Demonstrated strong verbal and written communications skills
Must be organized, detailed oriented and be able to prioritize tasks
Excellent customer service skills
Computer literate with Intermediate skills in Office 365 including Excel, Word and Outlook
Must be able to travel to all Pharmacy sites in a timely manner as needed
