Adult Foster Care of the North Shore (AFCNS) is seeking a full-time RN with at least 2-3 years’ recent experience in the direct care of elders or adults with disabilities in a family-based setting. The position will consist of half a caseload and the other half of the time completing intakes. The nurse will provide nursing assessments, developing and overseeing care plans, providing in-home training, and coordinating care and communicating with doctor's offices. The nurse will work in conjunction with the AFCNS Care Managers as part of a multidisciplinary team in order to provide supervision, training and case management via home visits of our members and caregivers.
The perks of this position is the flexibility! Employees make their visit schedule and mainly work remote as they are on the road conducting home visits. When not on visits, employees are expected to be working from their home office to complete notes and paperwork, checking emails, and all other job tasks that come along with the position.
The main office is located in Gloucester, MA. It is required to attend 1 staff meeting weekly in Gloucester. Travel for visits can be both Essex and Middlesex counties.
Qualifications:
-Current RN Massachusetts license
-Valid Massachusetts driver’s license and reliable transportation
-Past home-care experience
-Computer and typing skills a must. You should be comfortable with word-processing, scanning, emailing and learning our AFC database.
-Current knowledge of AFC MassHealth regulations a plus.
-Spanish speaking a plus.
