Summary of Position: The Case Manager plays a pivotal role in providing direct support and comprehensive case management to residents within our program(s). This position is crucial for maintaining residents' daily records and ensuring adherence to program policies and procedures. Collaborating closely with the Program Director, the Case Manager is instrumental in facilitating smooth operations and effective support mechanisms for our residents. Due to the type of clients this program serves, we are looking to hire female applicants only.

Job Functions:

Client Coordination: Coordinate client acceptance processes promptly, including reviewing referral packets, participating in interviews, and liaising with clinical teams and collaterals.

Intake Procedures: Complete all intake paperwork efficiently, ensuring immediate needs assessments, release forms, medication documentation, safety plans, and program policies are thoroughly reviewed and understood.

Treatment Planning: Facilitate comprehensive treatment plans, conduct regular reviews, and ensure all required assessments and documentation are completed in a timely manner, adhering to regulatory standards.

Communication: Maintain effective communication with DCF, school personnel, and other provider agencies regarding residents' services, treatment goals, and progress updates.

Resident Support: Provide leadership, act as a positive role model, and offer support to residents and staff in crisis situations or daily challenges.

Transportation: Arrange and provide transportation for youth under 18 years old to various appointments and meetings, ensuring their safety and well-being.

Continuous Improvement: Participate in Continuous Quality Improvement efforts, including maintaining, tracking, and documenting necessary information for program enhancements.

Training and Development: Conduct in-house trainings for staff, participate in professional development opportunities, and stay updated on relevant industry trends and best practices.

Discharge and Transition Planning: Oversee all communication regarding residents and their teams, facilitating smooth transitions and addressing any issues that may arise.

Interview and Intake Processes: Conduct interviews and intake procedures for new residents, ensuring compliance with program guidelines and documentation requirements.

Qualifications:

Bachelor's degree in psychology or related field preferred.

Minimum of two years of experience working in a residential setting or relevant field.

Ability to work flexible hours to accommodate client schedules and program needs.

Proficient in collaborative and proactive solutions for behavior management.

Strong communication and organizational skills are essential.
