MAP Site Supervisor

Schedule: Monday - Friday 9am - 5pm

SUMMARY OF POSITION

The Medical MAP Site Supervisor primary role is to oversee the state mandated Medication Administration Program’s. To ensure policies and processes are implemented and adhered to of the resident’s living at the ITR, CTR, Phoenix Rising, and Pre-Independent Living Programs. This includes but is not limited to ensuring MAP paperwork and medication administration reports/requirements are met daily, weekly and monthly. This position will require working collaboratively with the program Directors, Staff and providers to ensure participants' medical needs and program MAP requirements are met.

JOB FUNCTIONS

· Support MAP staff on the MAP State Requirements

· Support with Client Medical Needs and Program MAP Requirements.

· Conduct weekly assessments of residents’ needs

· Make Sure Medication Counts are completed in Narcotics Book for all MAP programs

· Medication Book Updated according to MAP Guidelines (see sample med book)

· Post and Verify orders new orders.

· Complete Medication Disposal with Staff when necessary

· Support Medical Case Manager with requesting any refills from Richmond and / or providers

· Review errors or questions found by MAP Staff

· Provide Oversight on the Transcription of any new medications with HCP Order

· Oversee the completion of MARs for the current and following months.

· Update resident Medication in Medsoft as needed.

· Assist in schedule and conduct map medication passes.

· Reviewing and submitting Medication Occurrence Reports

· Providing MAP On-Call ( in rotation with Medical Case Manager, and Nurse Consultant)

Medical Responsibilities

· Oversee the completion of Updated HCSP

· Work with Medical Case Manager on scheduling all needed resident medical appts

· Support clinicians with referrals and getting medical documents as needed

· Get support from clinicians to get new patient paperwork signed by DCF

· Oversee Update Med List on med soft

· Communicate with providers as needed (drug test updates, new symptoms, med refusals)

· Work with medical case manager with updating appts in Google Calendar

· Provide oversight on encounter forms for next day appointments (attach all needed documents fact sheet, insurance, consents, med list) File Encounter Forms

· Any other duties or assignments deemed necessary or relevant to the position as required.

QUALIFICATIONS

Valid driver’s license with an acceptable driving record required. Valid MAP certification is a requirement; additional education in health care is preferred. MAP Site Supervisor certification required; course can be taken upon hire.
