Role Description
This is a part-time on-site role for an Outreach Specialist at Comfort Keepers of Middlesex in Stoneham, MA. The Outreach Specialist will be responsible for community outreach, conducting research, writing communication materials, and providing exceptional customer service to clients and partners on a daily basis.
Qualifications
Strong Communication and Customer Service skills
Experience in Community Outreach
Proficient in Writing effective communication materials
Excellent interpersonal and relationship-building skills
Ability to work independently and in a team setting
Previous experience in the healthcare or social services industry is a plus
The ideal candidate will have a background in caregiving (CNA, HHA, LPN) and experience in client needs assessment
Responsibilities:
Caregiver Introduction: Facilitate the introduction of caregivers to their assigned cases, ensuring they are well-prepared to meet the client's needs.
Client Assessment: Conduct thorough assessments of clients to determine their specific needs and develop tailored care plans.
Assist in deciding what services and support each client requires, coordinating with caregivers to ensure these needs are met.
Support and Guidance: Provide ongoing support and guidance to caregivers, addressing any issues or concerns that arise.
Flexible Scheduling: Maintain a flexible schedule to accommodate the varying needs of clients and caregivers.
