The Homeless Prevention Council, in collaboration with the Town of Provincetown, is seeking qualified candidates to join our team of Case Managers. This person will work specifically in Provincetown, connecting Provincetown residents with mental health, substance abuse and other supports as needed.

Responsibilities for this position include:

Outreach about the service to previously unserved residents

Relationship building with unserved and underserved Provincetown residents

Assessment of individual need

Strength-based case management

Referral to and assistance with accessing services

Supporting in navigating services

Home visits and safety checks

Advocacy for addressing gaps in community health resources and services

Disbursement of funds for client expenses related to the provision of services (e.g. travel assistance to care appointments, food, etc.)

Meeting with clients and collecting all documentation required for a thorough intake of each client.

Providing comprehensive case management services including, but not limited to: crisis intervention, providing appropriate resources, tools, and counseling to assist participants in achieving their case plan goals; making referrals for services; following up to ensure that the referrals were completed and tracking and documenting participant progress.

Collaborating with other agencies and support resources to maximize participant outcomes and program goals.

Regular meetings (weekly) with the HPC Case Management team as well as ongoing collaboration and regular communication with the Town of Provincetown Health Department.

Entering all data into the HPC database and running reports as needed for the Town of Provincetown and others as requested.

QUALIFICATIONS:

Candidates should have experience in working with at-risk or underserved populations, particularly in the areas of Behavioral Health, Crisis Intervention, Social Work or Public Health. Experience in early intervention and screening to determine needs preferred. Bachelors Degree required. Proficient with Microsoft applications; word, excel and data base systems. Precise record keeping, excellent oral and written communication skills and a commitment to teamwork is essential. Bi-lingual English/Spanish speaking candidates are encouraged to apply.

Must have a valid MA driver's license and transportation. Evening and weekend outreach hours in local community locations will be required, including weekends. Preferred familiarity with the Lower and Outer Cape communities.
