Looking for a private pay Home Care Provider for a 74 y.o. woman with Parkinson's Disease. Applicant must have completed the free video-based Massachusetts Personal and Home Care Aide State Training (PHCAST) and have the Certificate of Completion OR present other more extensive certification. "S" is functioning well cognitively, is able to feed herself, and participate in many activities in the Westborough condo complex in which she lives with her husband. She is able to use an upright walker part-time, but also uses a manual wheelchair. We are seeking a provider who has no trouble understanding English and who speaks English at least moderately well. We need someone who has a clean driving record and can drive (we will provide an insured SUV) "S" to nearby shopping and appointments (most within 10-15 minutes from "S's" home). The provider must be able to fold and lift a 35 lb. manual wheelchair into and out of the SUV's tailgate. Help with routine household chores and "S's" hygiene activity including assistance showering, some toileting and dressing, doing laundry, assisting with cooking. Need approximately 28-32 hours per week with schedule to be negotiated (for M,W,Th,F,Sat), but no night-time and no Tuesdays or Sundays. "S's" husband is fit and covers Tuesdays, Sundays, and all evenings and overnights. Applicant must have at least a year of prior relevant experience. In addition to certification, we will need references and applicant must be willing to have us run a background check if that is not included in the certification the applicant presents. We will provide an employment contract and file all necessary State and Federal forms.
