RISE is a division of JRI Health that provides services to people in the Metro West area, Framingham. Our services include free confidential HIV counseling, testing and referrals, STI and Hepatitis testing, HIV Case Management, Housing Search & Advocacy, Peer Support Services, LGBT services, Overdose Prevention and Education, community education programs on HIV, STIs, viral Hepatitis, harm reduction.

RESPONSIBILITIES

Under the direction of the HIV Client Services Program Coordinator, the Bilingual English-Spanish Medical Case Manager will be responsible for providing multiple aspects of psycho-social care: intake, assessments, reassessments, acuity reviews, harm reduction/risk assessment, medication adherence counseling, and positive prevention counseling, HIV, STD, & hepatitis education, referral to medical & psycho-social services, advocacy & housing placement assistance.

QUALIFICATIONS

BA/BS in health education or Social Services related field preferred. Minimum 3 years experience in human services. Experience working with persons living with HIV/AIDS. First-hand knowledge of Hepatitis, STIs, HIV/AIDS, prevention and health care education. Knowledge of harm reduction philosophy and practice. Work experience / experience may be substituted for education. Strong interpersonal, verbal, and written communication skills. Strong case management & client record documentation and charting skills. Very good interpersonal and communication skills. Understanding of client-centered communication. Ability to deal with sensitive issues in racism, classism, homophobia, and sexuality. Bilingual in English-Spanish is a must. Fluency in Portuguese is strongly encouraged. A valid Massachusetts drivers license and a reliable vehicle. Own transportation required. Ability to interact with people from diverse backgrounds and communities, including LGBT (Lesbian, Gay, Bisexual, Transgender) individuals.
