General Description The Team Leader – Hospice assesses and identifies patient needs in accordance with the hospice concept of care. Coordinates with the Interdisciplinary Team to provide complete patient/family care. Maintains the continuity of care by ensuring that patient needs are met. Provides clinical supervision of assigned nursing staff. Coordinates referrals, admissions, and discharges. Reports to Hospice Supervisor/Manager. EDUCATION/EXPERIENCE REQUIRED Graduation from an accredited school of nursing.Enrollment in a Bachelor of Science in Nursing (BSN) program within 3 years of hire.Completion of a Bachelor of Science in Nursing (BSN) within 5 years of hire. Requires an understanding and acceptance of the Hospice concept of care, and a working knowledge of current pain and symptom management.Requires knowledge and experience appropriate to the needs of hospice patients and their families.Must meet or exceed core customer service responsibilities, standards and behaviors as outlined in the HFHS’ Customer Service Policy: Communication; Ownership; Understanding; Motivation; Sensitivity; Excellence; Teamwork; Respect.Must practice the customer skills as provided through on-going training and in-services.Must possess the following personal qualities: Be self-directed; Be flexible and committed to the team concept; Demonstrate teamwork, initiative, and willingness to learn; Be open to new learning experiences; Accepts and respects diversity without judgment; Demonstrates customer service values.CERTIFICATION / LICENSURES REQUIRED:Current Michigan licensure as a Registered Nurse.Requires a valid driver license, an acceptable driving record, and reliable transportation.PREFERRED REQUIREMENTS:Hospice certification preferred.Additional Information
