POSITION SUMMARY: Functions as a core member of an integrated behavioral health team that may involve the patient’s primary care provider in a clinic and/or school-based setting while adhering to all applicable state and federal confidentiality requirements. Works under the direction of licensed behavioral health and/or medical providers, is responsible for coordinating referrals, and supporting behavioral health services as outlined in the patient’s treatment plan. Supports and coordinates behavioral health services with the patient’s integrated care team.

Assists the integrated care team in collecting information, documentation and/or screeners to be used to determine the patient’s behavioral health needs.

Engages with patients during warm hand-offs to offer support, education about integrated care, resources, and behavioral health intake processes. Assists with scheduling for initial and follow-up visits.

Assists patients to access services using approved multi-media platforms.

Documents encounters in the EHR per One Health Policy, Procedures, and Guidelines.

Facilitates referrals for clinically indicated services specified on the patient’s treatment plan.

MINIMUM JOB QUALIFICATIONS:

Education/Certificates/Licenses:

Required

Preferred

Bachelor’s or Associate’s degree in a related field from an accredited college or university, mental health aide, behavioral health aide, medical assistant, licensed practical nurse, or nursing assistant

Experience:

Experience in a medical clinic or mental health setting; or

A n y combination of experience and training which provides the equivalent scope of knowledge, skills, and abilities necessary to perform the work; including the ability to communicate effectively with others, and work under the direction of a licensed behavioral health and/or medical provider

  • SUMMARY OF JOB PURPOSE AND FUNCTION The primary purpose and function of the Practice Manager is to manage the hospital and ensure the highest quality of veterinary care, exceptional client service, associate engagement and maximum productivity of the veterinary medical team. Train the team on Banfield guidelines and practices and supervise associates to ensure compliance.  Regularly supervise the team of paraprofessionals in the hospital, consisting of at least two associates and often more (the size of the paraprofessional team will vary by hospital depending on factors such as business volume and client needs).  Oversee and train associates on professional, efficient and exceptional client service (lead by example), to include client education about Optimum Wellness Plans, preventive care, pet health needs, hospital services, marketing campaigns, and other related information; effectively resolve client issues.  Follow all Banfield protocols and practices and be responsible for ensuring that associates achieve practice priorities while building our culture and brand.  Identify potential “bottlenecks” and formulate solutions to remove barriers in medical operations that would impede the doctors from providing world class care.  Partner with the Field Director and Chief of Staff (if applicable) to interview, select, train, develop, coach, mentor and, if necessary, discipline and terminate paraprofessional staff.  Direct, lead and mentor the paraprofessional staff to realize their full professional potential and deliver outstanding, high quality medical care, client service and business results.  Work with the Field Director to establish development plans (PDPs) for the paraprofessionals, and focus teams to deliver the practice priorities and exceed goals while building our culture and brand.
