The Care Manager is a key member of the Home Care Department at Circle of Care St. Louis. This role includes conducting weekly home visits to clients and ensuring personalized and comprehensive support is provided. Responsibilities include managing client care plans, performing home care assessments, facilitating access to necessary community resources, and submitting referrals for Home and Community-Based Services. The Care Manager also educates clients and families on medication management, ensuring treatments are understood and adhered to, contributing to overall health and wellness.Key Responsibilities: Client Intake and Assessment: Conduct initial intake interviews with new clients to gather vital information, assess their needs, and develop appropriate service plans. Home Care Assessments: Analyze clients' living environments during home visits to determine the need for additional services. Medication Education: Educate clients and their families on medication management, ensuring they understand their treatments and the importance of adherence to promote optimal health outcomes. Resource Management: Research, compile, and maintain an updated directory of local community resources and services, such as food banks, housing assistance, and job training programs. Professional Development: Engage in ongoing training sessions to stay informed about the latest resources, social services procedures, and best practices in client care. Qualifications: Registered Nurse (RN) or Licensed Practical Nurse (LPN) with a current license valid in Missouri. Proven experience in healthcare or social services settings, particularly in a community-based role. Exceptional interpersonal skills with a proven ability to empathize with and advocate for clients. Outstanding organizational skills and meticulous attention to detail. Proactive and resourceful with a strong ability to resolve problems and manage multiple priorities. Proficient in using medical and office software for documentation. Working Conditions: This role requires traveling to clients' homes; reliable transportation is necessary.
