Circle of Care in St. Louis is actively searching for a Registered Nurse (RN) to join their team part-time, dedicating 10-12 hours per week to providing at-home care. This caregiver role encompasses a flexible schedule, allowing you to set your hours!Key Duties: Regular Home Check-ups: Offer care to 10-12 clients per week in their homes, tailoring your schedule to suit their needs and your own. Medication Management: Assist with weekly pre-filling of insulin syringes and daily setups of oral medications for eligible clients. Nail Care: Offer monthly in-home nail care services to qualifying participants. APC Plan Assessment: Routinely perform in-home evaluations of advanced personal care services, ensuring they meet quality and appropriateness standards. APC Aide Training: Provide real-time, on-site training for APC aides as required. Discharge Consultation: Attend discharge meetings to ensure clients enjoy smooth transitions from nursing facilities to their homes. Ideal Candidate: Holds a current RN or LPN license in the State of Missouri. LPN candidates should have at least one year of experience caring for the elderly, individuals with disabilities, or those having complex medical conditions. Has a minimum of 2 years of experience in providing care to the elderly, individuals with disabilities, or those having complex medical conditions. Capable of conducting home visits and delivering high-quality, person-centered care. Ready to provide preventive and maintenance care services not intended to treat acute health issues. Experienced in ensuring seamless transitions for clients moving from skilled nursing facilities to a home setting. Excellent communication skills and ability to provide culturally sensitive, personalized care to a diverse client base. Includes providing in-home nursing services of a preventative or maintenance nature to participants with stable, chronic conditions.
