The starting rate for this position is: $21.55 Rate of pay displayed reflects the beginning of the pay scale. Job Summary Renders nursing care to patients within an assigned unit of the hospital, in support of medical care as directed by medical staff, RNs and LPNs, and pursuant to objectives and policies of the hospital. Essential Duties and Responsibilities

Supports and promotes the mission and values of Covenant Health Ministry.

Practices according to the ANA guidelines of care as adopted by the Nursing Division.

Adheres to the Nurse Practice Act of NH.

Performs various patient care activities and related services necessary in caring for the personal needs and comfort of patients.

Documents all care provided to the patient in the Electronic Health Record. Such as Vital Signs, Intake, Output, Mobility, pain, and skin abnormalities. As well as important information related to interactions with the patient and/or visitors.

Reports any unusual or changes in patient condition to the primary RN and/or charge RN

Completes bedside shift hand-off report routinely and independently.

Answers call lights to determine patients’ needs in a timely manner. Understands and demonstrates initiatives for improved response times to our patient’s needs such as NO PASS ZONE.

Performs bladder scans, EKG’s, CPM’s, assists with cold therapy and other appropriate medical procedures upon completion of appropriate competencies.

Bathes patients daily and assists with personal hygiene to increase their comfort and well being.

Encourages and assists all patients daily with routine ambulation and mobility, unless otherwise ordered by the attending provider or instructions by the primary RN, and documents activity in the Electronic Health Record.

Serves and collects food trays, assists with intake for patients requiring help, and provides between meal nourishment.

Collects samples for testing i.e., urine and fecal occult.

Assists with specialty beds and mattresses

Assists with skin and wound care under the direction of a nurse.

Expresses interest in the patient and their progress, assuring patient needs are met.

Uses judgment in providing information based on the situation and is sensitive to individual and organizational concerns.

Demonstrates effective verbal and non-verbal communication technique.

Provides a secure and safe environment for behavioral health patients after completing appropriate competencies for one to one, constant observation, and continuous Visual remote monitoring.

Performs continuous visual monitoring of patients, initiated by the primary RN, upon completion of assigned competencies related to equipment and role responsibilities.

Explains any nursing care given to involve the patient in their care.

Demonstrates willingness to rotate among various shifts or departments to adjust staffing to meet patient care needs.

Assists with performing in the Hospital Unit Coordinator role as necessary to meet the needs of the unit.

Prepares equipment and assists other nursing staff during treatments and examination of patients.

Uses equipment/safety protocols and policies in providing care and transport to patients.

Demonstrates understanding of the fall prevention measures for all patients assessed by the RN as high fall risk. Communicates with the RN and provides report to on coming LNA any concerns.

Utilizes AIDET when interacting with patients and assists with providing hourly rounding to all patients.

Completes all assigned annual competencies by due dates.

Maintains adequate patient care hours for licensure.

Maintains current license and certifications required at all times.

Avoids use of unauthorized overtime.

Manages ETO (Earned Time Off) per policy

Other duties as consistent with this role.

Job Requirements: (education and experience/preferred and required)

Job Knowledge and Skills

Crisis Prevention Institute (CPI) training preferred or obtained prior to completion of orientation

Education and Experience

High School Graduate or GED preferred

Current Hampshire LNA license required

BLS/CPR AED Certification required

An equivalent combination of education and experience which provides proficiency in the areas of responsibility listed above may be substituted for the above education and experience requirements.

Working Conditions/Physical Demands

Must possess the physical and mental abilities to perform the tasks normally associated with this position that involves a combination of sitting, standing, walking, bending, stooping, and reaching. Lifting/carrying frequently up to 35 pounds, and occasionally up to 50 pounds. Some stress related to high level of responsibility for quality care.
